Johnson County Child Support Application Information To open a case with the Johnson County Child Support Division, you must obtain, and complete, a Child Support Title IV-D Application for review by an Enforcement Agent. This application may be obtained at the Child Support Division Office, located at 1 Caisson Drive, Suite A, Franklin, Indiana 46131 or by calling the Johnson County Child Support Intake Line at (317) 346-4584 and requesting an application be mailed to your address. In order to receive the services through the Johnson County Child Support Division, it is important that all required information be provided by the person applying for services on the application. During the application process, some requested information may seem overly personal. Please keep in mind that this information is necessary to provide the best possible services in child support enforcement. TEMPORARY ASSISTANCE FOR NEEDY FAMILIES (TANF): If you are receiving TANF, there is no cost for child support enforcement services, and upon receipt of TANF benefits, your case should automatically be referred to the Johnson County Child Support Division office for applicable enforcement. MEDICAID : If you are receiving Hoosier Healthwise/Medicaid benefits, you are entitled to the services of the Johnson County Child Support Division at no cost. However, you must specifically request enforcement of child support services by in writing. The cases in the Child Support Office are divided by the name of the child support payor's last name.
