HOW TO COLLECT ON BAD CHECKS:

* Complete the enclosed NOTIFICATION FORM and mail "certified mail-return receipt requested" to the person who signed the check. Keep a copy for your files. The U.S. Post Office will return the signed "green card" or the unclaimed letter to you.

  1. If the unclaimed letter is returned to you, make the following copies and forward to the Prosecutor's Office.
  2. If the "green card" is returned to you, wait ten (10) days for the check writer to submit payment in full. If no payment is submitted, make the copies of the documents listed under #3 below, and forward them to the Prosecutor's Office at:

* Send us the following documents:

  1. THE ORIGINAL and 1 COPY of a completed and signed Bad Check Affidavit sheet, to be submitted for the deputy prosecutor to review and sign.
  2. 1 COPY of front and back of the bad check; ( DO NOT send the original bad check)
  3. 1 COPY of the NOTIFICATION FORM sent to the issuer of the check
  4. 1 COPY of the "green card" or unclaimed letter from Post Office
